We ask that you bring your current and complete insurance information with you so claims may be filed on your behalf. We will make every effort to maximize your insurance benefits. We accept most traditional insurance plans; however, we are participating or in-network with Delta Dental Premier and Aetna only. We do not accept any of the Medicare or Medicaid plans..
Payment and Financing
To keep dental costs down while maintaining a high level of professional care, we ask for payment in full when services are rendered. All major treatment and treatment involving laboratory procedures will require an appropriate down payment. Any co-payment and/or deductible amounts are due at the time of service.

Without 24-hour notice, a fee of $25 will be charged for a broken hygiene appointment and a fee of $50 will be charged for a broken appointment with Dr. Kelly. The fee must be paid before the appointment can be rescheduled. We appreciate your cooperation and understanding.
